Description of Services:

Program assists people living with dementia and their families make informed choices for better quality of life

Services may include:

assistance with identifying and accessing information, supports and services that meets needs
facilitates connections to education, programs and services within the organization as well as other health care and community service providers
one-on-one counselling
caregiver support groups
support groups for persons newly diagnosed
referral to community resource by phone, drop in, or home visit
