Bereavement support for anyone who has experienced the death of someone close * services can be provided at home, or in hospitals and long-term care homes Bereavement Services - Support groups held in various communities throughout the year |

Eligibility - Population(s) Served: | People who need end-of-life care or bereavement supports * must live within geographic service of hospice (Steeles Avenue to the Lakeshore, Yonge Street to Victoria Park) * 18 years of age or older | ||
Application: | Call * no referral required * referrals from health care professionals require completion of the Palliative Care Common Referral Form | ||
