Description of Services: A 16-week program that assists individuals to transition home when you are discharged from Mackenzie Health
  • offers direct care services based on the assessed requirements for each patient, which might include a personal support worker (PSW), and rehabilitation supports such as physiotherapy and occupational therapy, and caregiver support/relief as required (social worker)
  • Care Navigators oversee each patient's transition plan and recovery after discharge from the hospital * community support services may include transportation, meal on wheels, coordination with the patient's family physician, setting up safe and appropriate in-home care

Eligibility - Population(s) Served: Patients must live within service boundaries and assessed by Care Navigator to determine eligibility
