Description of Services: Dedicated inter-professional primary care team supporting complex homebound clients residing in the west end of Toronto * team comprised of an Ontario Health atHome care coordinator, physician, nurse practitioner and pharmacist * provides medical care in the home to clients living in designated catchment area and who cannot get out regularly to see a family doctor * this west-based Toronto team will work together with clients and families to help manage a client’s health care needs

Fees: Free * no Ontario Health Insurance required for general services
Eligibility - Population(s) Served: Home-bound due to chronic health conditions and unable to access primary care * 65 years of age or older * valid OHIP coverage
