Community-based program that focuses on serving seniors who are experiencing behavioural challenges * works in collaboration with hospitals, specialized teams, and primary care providers

Services include:

transition plan development (from current environment to supported care environment)
resources and support services
psychosocial assessments
mobile social work
risk identification
case coordination
therapeutic intervention
