Description of Services:
Transition support from school life to adulthood for those with developmental needs 

Program initiatives:
  • support of a coordinator for the participant and their family as well as for the other agencies that are involved
  • assessment of skills and abilities 
  • use of the Person Directed Planning model in creating and maintaining goals for the future
  • customized supports and resources based on needs and interests
