Description of Services: A six-month treatment agreement which facilitates an intensive community-based care plan for individuals with a serious mental illness. The focus of the agreement is to provide support, case management, maintain wellness, and reduce hospitalization. The treatment agreement is voluntary and treatment orders can be renewed after the six-month period. Admission Criteria: The patient must have had at least two admissions in the last three years. Or an admission of 30 days or more in the past three years.
