Description of Services: Provides individualized support to clients living with age related dementia and clients over 65 living with a serious mental illness. Supports include: - comprehensive in-home safety assessments - regulate safety checks/monitoring - collaborative development of a personalized safety plan - assistance to achieve/maintain optimum health and wellness

Eligibility - Population(s) Served: Older Adults Only
Application: Referrals are accepted from anyone, including self-referrals. Referrals are through The Access Point. Please call or fill out the application form online. Once the application is received, a service navigator will call the individual to set up an appointment to do an intake assessment.
