Description of Services: Short term psychogeriatric services to older adults and their care partners, with mental health and/or dementia concerns * visits by community clinicians (Social Work, Occupational Therapist, Registered Nurse and Nurse Practitioner) to complete a comprehensive psychogeriatric assessment, treatment planning, advocacy, education and referrals to other services as required * home visits, via Ontario Telemedicine Network (OTN), with Geriatric Psychiatry * follow up as required

Eligibility - Population(s) Served: Age 55+ with mental health and/or dementia concerns living in the community
Application: Referral Form required; referrals accepted from health service providers, self referrals and/or General Practitioners
