Description of Services: Hospital-based outpatient program for older adults who have concerns about their physical health or memory * provides referrals to Baycrest programs and services as needed * services provided by geriatricians and other healthcare professionals

Geriatricians will be able to: 
  • assess your physical and mental health 
  • recommend a personalized treatment plan 
  • refer you to the appropriate specialty clinics, programs and support services at Baycrest or elsewhere, as needed 
  • provide recommendations to your family doctor to assist in coordinating your care plan


Fees: No fee associated for those seen at the clinic
Eligibility - Population(s) Served: Older adults seeking to assess their health care plan and have been referred by a physician
Application: Referrals from a physician or nurse practitioner for this service can be initiated through our Central Navigation, which adheres to a no-wrong-door approach. This assists community agencies, primary care providers, and other healthcare professionals in navigating Ambulatory Services at Baycrest.
