Program: Caregiver Support

Organization: Hospice Vaughan
Description of Services: Support for those caring for a loved one at home or from afar:
  • develop and evolve your personalized care plan with you
  • provide in-home social visits from specially trained volunteers
  • participate in individual counselling and/or support groups (based on community need)
  • assist you with referrals and in connecting with other community caregiver services
  • support you in the transition to grief and bereavement supports
  • participate in complementary therapies and wellness activities (e.g. yoga, reiki, etc)
  • participate in educational workshops (based on community need)
