Program: Pulmonary Rehabilitation Program and COPD Clinic

Organization: Vaughan Community Health Centre
Description of Services: Supports people at risk or living with Chronic Obstructive Pulmonary Disease (COPD) * clients will have access to a respiratory therapist/certified respiratory educator, registered kinesiologist and registered dietitian * the goal of the program is to improve the quality of life for people living wih COPD

Services include:
  • breathing test
  • one-on-one counseling (including self management education, healthy eating, physical activity)
  • smoking cessation program
  • individual and group exercise
  • pulmonary rehabilitation
  • COPD Virtual Care Groups - twice a week
  • COPD Support Group - virtual/in-person on last Tuesday of each month from 1:30pm-3pm

Fees: Free
Eligibility - Population(s) Served: People at risk or living with COPD over the age of 18 years old
