Program: Geriatric Mental Health Outreach Team

Description of Services: Team works with residents, family, staff and other health care professionals at individual long-term care homes to:
  • manage/treat challenging behaviours and mental health / psychiatry concerns at site
  • avoid unnecessary visits to the Emergency Department
  • ensure safe transfers to and from the hospital, when appropriate
  • enhance support and build knowledge capacity, as needed
Provide the following specialized services for residents: assessment, consultation/ Treatment and short-term follow-up

The team is made up of Advanced Practice Nurse, Nurse Clinicians with expertise in the area and Psychogeriatrician (a specially-trained doctor in this area)

  • Chartwell Gibson Long Term Care and Retirement Home
  • Cheltenham Care Community
  • Cummer Lodge
  • Downsview Long-Term Care Centre
  • Extendicare Bayview
  • Harold & Grace Baker Centre
  • Hawthorne Place Care Centre
  • Norfinch Care Community
  • North York General - Seniors Health Centre
  • Valleyview Residence
  • Villa Colombo
  • Village of Humber Heights
  • Weston Terrace Care Community
