Description of Services:
The Supportive Care Team is composed of professionals who specialize in providing emotional support, education, and compassionate care for people living with a serious illness, their caregivers, and those grieving a loss
Support provided to:
Serious Illness Support - for individuals and families in the community coping with serious illness, caregiving, and loss
Support provided to:
- anyone living with a serious illness
- caregivers and family members
- anyone grieving a loss
- education and guidance to help caregivers feel prepared
- emotional support and counselling
- connections to community resources
- caregiver support groups
- share their feelings and experiences
- learn about grief and ways to cope
- connect with others who understand
- receive individual or *group support
- HUUG - Help Us Understand Grief
Serious Illness Support - for individuals and families in the community coping with serious illness, caregiving, and loss
- emotional support and counselling through difficult times and life changes
- help understanding options and making decisions
- support navigating healthcare systems and community resources
- education about illness, end-of-life planning, and grief
- support with advance care planning and legacy work
