Application
Referrals are accepted from anyone, including self-referrals. Referrals are through The Access Point. Please call or fill out the application form online. Once the application is received, a service navigator will call the individual to set up an appointment to do an intake assessment.

Description of Services:
Offers 24-hour high-support housing to individuals who require support in various areas such as activities of daily living (e.g. cooking, cleaning), accessing services, skills teaching, socio-recreational activities and managing their medications.
