Description of Services: This program helps inpatients transition back to home, school and the community. Program provides: medication assessment, diagnostic clarification, crisis intervention and stabilization of the individual by an interdisciplinary team consisting of a psychiatrist, social worker, nurse, child & youth worker, three teachers, educational assistant and other disciplines as required. Patients attending the program will participate in the classroom to promote academic achievement while receiving therapy.

Eligibility - Population(s) Served: Children Only ; Youth Only
Application: Referrals accepted from the inpatient unit, the outpatient department, the Emergency Department, family physicians, paediatricians, schools and the Children`s Aid Society. Individuals in this program are required to maintain abstinence from substance use while in the transition program. Medical clearance in the form of a doctor`s note is also required in order to be admitted to the program. Referrals can be faxed to intake after which an appointment will be scheduled.
