Description of Services: Provides permanent and affordable housing to individuals living with mental illness, enabling them to live independently and improve the quality of their lives. Support is provided to help individuals maintain their housing, and access the community resources which will assist them in their recovery process. Eligibility criteria: - Individuals who are 16 years of age and older and are residents of Toronto - Individuals who have been diagnosed with a serious mental illness - Individuals who are homeless or at immediate risk of being homeless and are able to live independently
