Description of Services: | Provide the treatment, rehabilitation, and support services that assist people with severe mental illness in their recovery and their desire to live in the community. The teams are multidisciplinary each having a social worker, nurses, a vocational specialist, an occupational therapist, a psychiatrist, a peer support specialist, and an addictions specialist. | ||

Application: | The East Intake coordinates access for the New Dimensions, East Metro and Scarborough Hospital ACT teams in Scarborough. Referrals are made by contacting The Access Point. | ||
