Geriatrics: Geriatric consultation for older adults experiencing functional decline (cognitive and/or physical). Service is supported by multidisciplinary team (Geriatrician, Nurse Practitioner, Nurse, Social Worker, Occupational Therapist) * Referrals are triaged to the most suitable service * Consultative model, report and recommendations communicated with the primary care * Not intended for ongoing monitoring Non-Malignant Palliative Care: Supportive care for individuals with advance, non-malignant illness where ongoing disease progression is expected, or for those exhibiting markers of poor prognosis. * Assist patients in understanding their anticipated illness trajectory and preparing for its progression * Not intended to support chronic pain or chronic disease management Movement Disorders: Consultative care for individuals with a presumed or established diagnosis of a movement disorder * support for pharmacological optimization |
