Description of Services:
For patients over the age of 75, consultation by geriatric nurse within a hospital emergency department to provide assessment, diagnosis, identification of frail and at risk elderly, and to initiate appropriate treatment in the hospital or community. Focus is centered around appropriate goal setting for independent community living
