Description of Services: Perform in-home comprehensive geriatric assessment of age-related issues, and offer solutions to help seniors maximize their health, independence and quality of life * referrals and linkages to community and health care resources are carried out by an inter-professional team, within a person's home * Note: Our geriatrician does not do home visits

Fees: Service covered by OHIP
Eligibility - Population(s) Served: Refer yourself or have a doctor, health care professional or family member refer you.
Application: Referral by a physician is required * the clinic will accept patient referrals from Emergency department, post discharged and the community * will consider other referrals that do not meet age criteria
