Description of Services: Conducting assessments, planning, coordinating services and ongoing monitoring to meet the needs, goals and expected outcomes for those living with Alzheimer's disease or other dementias and their caregivers * provide education around the disease process * help connect to other services in the community * one-on-one counselling to provide emotional support to caregivers * First Link Caregiver education series run across the region
