Description of Services: Supports for people living with prediabetes or type 2 diabetes * one-on-one or group consultation on monitoring blood sugar levels, medications, healthy eating, exercise, and other lifestyle choices * registered nurse and dietitian on staff * chiropody services available with referral from your physician or diabetes educator
  • weekly virtual or in person groups - for people new to diabetes or a refresher
  • insulin starts
  • glucose screening clinics
  • nutritional counseling (healthy eating, meal planning recommendations)
  • post-gestational
  • foot care and chiropody services
  • weight management
