Description of Services: Program coordinator will provide a one-time individualized consultation for care partners and people with Parkinson's disease (PD) * assist them to optimize benefits of Parkinson's medications, to recognize their motor and non-motor symptoms related to Parkinson's

For those who are also able to participate in group education, will be invited to join our 8 weekly education and exercise program * objective is to learn strategies and healthy lifestyle to live well and to delay disease progression * presentations provided by our inter-professional team including physiotherapist, pharmacist, occupational therapist, recreational therapist, dietician, nurse practitioner and social worker

Fees: One time individualized consultation is free, $100 for the group education (free for care-partners)
Eligibility - Population(s) Served: People with Parkinson's disease, their family and care partners
Application: Referral and consultation notes from doctor is required
