Contact the Developmental Services Ontario (DSO)- Central East Region to apply for services and support * Client and family/primary caregivers must fill out and sign an Application for Developmental Services and Support(ADSS)form

Eligibility / Target Population

Adults and children with developmental disabilities * All applicants will be assessed by the Developmental Services Ontario (DSO) to determine eligibility with the Ministry of Community and Social Services requirements

Fees

Fees may apply for some services * Annual Membership for individuals: $10
