Description of Services:

Education and consultation to service providers offering end-of-life care * sessions includes basic and advance palliative-care training * consultation regarding pain and symptom management

Services include:

case-based palliative care consultations available in-person, by phone or via email for providers
training and mentorship for front-line providers such as home care agencies, long-term care homes, hospices, day programs, supportive housing, correctional facilities, or volunteer providing hospice palliative care (courses and workshops designed to enhance knowledge)
palliative care resources on a variety of topics including advance care planning, goals of care and health care consent, caregiver resources, disease specific, end-of-life, grief and bereavement resources, palliative approach to care, palliative care service delivery resources, pain and symptom management
