A multidisciplinary team that specializes in caring for Seniors in the community. The team provides in-home non-urgent assessments to frail seniors that are unable to attend outpatient clinic appointments. The assessment focuses on functional, psychological, cognitive and physical health
Patient exclusions:- clients residing outside the Ontario Health atHome, Brampton (Central West) geography
- clients with no family physician
- clients under 65 years of age
Services offered:The team will conduct an assessment as related to the needs of the patient and/or issues that are identified such as; frequent falls; mobility concerns, medication polypharmacy, pain management, weight loss/nutrition, cognition/dementia, delirium, depression, anxiety, wandering, lives alone, caregiver stress, elder abuse, social isolation, ADL/iADL decline, home safety
