Description of Services:
Provides individuals and their families with information and support in accessing mental health case management, assertive community
treatment teams (ACTT), early psychosis intervention programs (EPI) and supportive housing in the city of Toronto.
The Access Point manages the centralized wait list for community-based individual supports and supportive housing across the city of Toronto. The Access Point provides information, assessment, service matching, wait list support and referral to mental health and addiction services. The Access Point is a collaboration led by LOFT Community Services.
